2. Appointment Scheduling & Cancellations
Please provide 24 hours’ notice for cancellations or rescheduling. Repeated no-shows may result in limited access to future appointments.
3. Communication & Text Messaging
By providing your contact information, you consent to receive appointment reminders, care-related updates, and rare promotional messages. Text messaging is not HIPAA-secure; by consenting, you accept the associated risks. Opt out any time by replying “STOP” or contacting us.
4. Financial Policy
Patients are responsible for co-pays, deductibles, and balances not covered by insurance. Insurance plans vary widely, and it is the patient’s responsibility to verify whether their specific plan includes in-network benefits for our services. Payment is due at the time of service. Accounts with unpaid balances may be sent to collections.
5. Privacy & Medical Records
We follow HIPAA regulations to protect your privacy. You may request access, corrections, or file complaints about your health information.
6. Medication & Prescription Policy
Prescriptions are provided when medically necessary and in compliance with legal regulations. Pain agreements and drug screenings may be required. Lost prescriptions are not replaced.

Respectful behavior is expected at all times. Harassment or noncompliance may result in termination of care.
8. Limitation of Liability
We are not responsible for outcomes due to non-compliance, misuse of services, or information obtained through our website without direct consultation.
