MetaCare is a patient-access mobile application provided by Metamorphosis LTD, doing business as Metamorphosis Pain Management (“Metamorphosis,” “we,” “us,” or “our”). This policy explains how information is handled when you use MetaCare.
This mobile-app policy supplements, and does not replace, our Notice of Privacy Practices and other patient privacy notices. Protected health information is also handled as described in those notices and as required by applicable law.
Information MetaCare handles
When you securely sign in, MetaCare may retrieve and display information from our Canvas patient-record and patient-portal services, including:
- your name, contact information, patient identifiers, and account information;
- appointments and follow-up status;
- medications and medication information reported by you or your care team;
- assigned questionnaires and assessment status;
- signed or released documents;
- insurance coverage, claim progress, balances, and payment status; and
- other information made available to you through your patient account.
MetaCare also stores limited information on your iPhone:
- authorization credentials are stored in the iPhone Keychain;
- your optional Face ID and reminder preferences are stored on the device; and
- scheduled notification information is stored through Apple’s notification services on your device.
MetaCare does not request access to your contacts, photos, microphone, precise location, or Apple Health data for the features currently offered.
How information is used
We use information handled through MetaCare to:
- authenticate you and provide access to your patient information;
- support treatment, payment, and healthcare operations;
- show appointments, forms, records, billing, insurance, and follow-up status;
- provide reminders you choose to enable;
- respond to support, privacy, or account requests;
- maintain security, prevent misuse, and troubleshoot the service; and
- comply with legal, regulatory, and professional obligations.
Notifications
If you enable notifications, MetaCare may remind you about upcoming appointments, assigned forms, or follow-up needs. Lock-screen notification text is intentionally general and does not include medication names, assessment names, procedures, morphine milligram equivalent values, balances, or other medical details. You can turn reminders off in MetaCare or in iPhone Settings.
Payments
When you choose to make a payment, MetaCare opens Canvas’s secure patient payment page. Canvas and its payment processor handle card entry and payment processing. MetaCare does not receive or store your complete payment-card number.
When information may be shared
We may disclose information:
- to Canvas and other service providers that help us operate our clinical, patient-access, security, and payment systems;
- for treatment, payment, and healthcare operations as permitted by law;
- when you authorize or direct us to disclose it;
- when required by law or a valid legal process; or
- when reasonably necessary to protect a person, our patients, our services, or our legal rights.
Service providers are expected to protect information consistently with their contracts and applicable law. MetaCare does not sell patient information, use it for third-party advertising, or use it for cross-app tracking.
Retention and deleting the app
Deleting MetaCare removes the app and its locally stored information from that iPhone. It does not delete information in your medical or billing record. Medical and business records are retained as required by law and our retention policies.
To request deactivation of your MetaCare or Canvas patient-portal access, contact either office during normal business hours. Deactivating portal access does not delete medical records we are legally required or permitted to retain.
Security
We use administrative, technical, and physical safeguards intended to protect information. MetaCare uses encrypted network connections, device Keychain storage for authorization credentials, and optional Face ID access. No security method can guarantee absolute protection. Protect your device passcode and do not share your patient-account credentials.
Your privacy rights
Depending on applicable law, you may have rights to request access to or copies of your information, request an amendment, request restrictions, request confidential communications, receive an accounting of certain disclosures, or submit a privacy complaint. Exercising a privacy right will not affect your care. Contact us using the information below. Additional details appear in our Notice of Privacy Practices.
Children and proxy access
MetaCare is intended for authorized patients and authorized caregivers or proxies. A caregiver or proxy must use access specifically granted to that person and should not use another person’s credentials. Access involving minors is governed by applicable law and Metamorphosis policies.
Changes to this policy
We may update this policy as MetaCare changes. The current version will be posted on this page with its effective date. Material changes will be communicated when required by law.
